There is a bug in last update Amdroid 4.1.2 that affects phones unlocked by imei of some models (N7000, N7100 I9100, I9300, etc).
  
This update relock again phone, and your unlock code not will work again. Only solution is upgrade phone using ODIN for not loose unlock of your phone.
 
Codes comes directly from factory, so claims after this are not accepted.
